Case Management

Navigating education, employment, healthcare, housing, and personal challenges can be overwhelming—but you don't have to do it alone. At Youth RISE Foundation, our Case Management program provides individualized guidance and resource navigation for youth and young adults ages 15–25.

Whether you're working toward graduating from high school, enrolling in college, finding stable housing, securing employment, or accessing healthcare, our case managers are here to help you identify your goals and connect you with the right services.

Our approach is strengths-based, person-centered, and focused on empowering every participant to build independence and long-term success.

Our Case Management team can assist with:

• Goal setting and individualized success planning 
• Referrals to housing and emergency assistance 
• Healthcare and mental health referrals 
• Food and basic needs assistance 
• Educational planning 
• Employment and workforce resources 
• Financial literacy programs 
• Government benefits navigation 
• Transportation resources 
• Community referrals 
• Crisis intervention and support 
• Ongoing follow-up and progress monitoring 

Case Management services are available to youth and young adults between the ages of 15 and 25 who are seeking guidance, support, or referrals. Whether you're facing a temporary challenge or planning for your future, we're committed to helping you find the resources you need.
